In-Home

In-home assessment is a service provided for the elderly and disabled to assess their level of functioning and independent living so that they can be cared for in the home environment instead of a care facility. A detailed assessment is made of physical and mental capabilities, medical status, social function and living arrangements. Your ability to perform specific activities called activities of daily living (ADL), including preparing and eating food, bathing, grooming and getting dressed, are key factors that are assessed. Recommendations are made based on the assessment for in-home services, healthcare referrals, assistive devices, home modification, fall prevention, exercise and management strategies for dementia and incontinence.
